So that we can get you checked in on time, we ask that you arrive at least 10 minutes prior to your scheduled appointment time. For your convenience, we provide downloadable forms for you to fill out paperwork ahead of your appointment.
Please give 24-hour notice for all appointment cancellations. You may be charged if you do not provide 24 hours notice of cancellation or if you miss your appointment. We have many patients who need that appointment and can be scheduled in your place if you provide the appropriate notice.
If you must cancel within 24 hours or miss an appointment, the following charges may incur as follows:
- $50 New appointment or Return appointment
- $100 Mohs or Excision surgery appointment
These charges are necessary due to the fact that we have many patients who need that appointment and can be scheduled in your place if you provide the appropriate notice.
Our office strives hard to see our patients in a timely manner. Therefore, if you arrive 10 or more minutes late, we may have to reschedule your appointment.
Please bring a photo ID and an insurance card to your appointment. We also need the social security number of the guarantor so we can file the insurance claim. Failure to provide this information will require payment for the full cost of the appointment and any procedures performed at the time of service.
We accept all major commercial insurance carriers, including Medicare. However, we do not accept Tenncare, Amerigroup, or Medicaid.
If the patient’s insurance plan requires a referral it is the patient’s responsibility to confirm that their primary care provider has obtained a referral prior to your visit.
Before patients arrive in our office, we verify insurance coverage. If the benefits cannot be verified, the full charge of the visit must be paid before leaving the office. Although we provide this as a courtesy, it is still the patient’s responsibility to check with their insurance plan regarding their coverage. Additionally, it is important to confirm that the specialist, with whom you have an appointment, is in network with their plan.
Self pay patients will be required to pay a $300.00 deposit upon arrival by cash or credit card. If your visit is more than $300.00, we require that you pay your balance in full upon check-out.
All cosmetic procedures must be paid in full at time of service.
No matter what special treatments your physician may use, remember that you must continue to use proper skincare until the tendency to have acne has passed. There is no instant or permanent cure for acne, but it is controllable and proper treatment may prevent scars.
Minors must be accompanied by their legal guardian during their first visit. After the first visit, a parental waiver must be signed if their guardian will not be present during future appointments.
If you are transferring your care from another physician to our practice, please have your medical records faxed to our office prior to your visit. This will prevent any confusion regarding previous treatments.
All patients who have biopsy performed are called back within one to two weeks with their biopsy results. If a patient has not heard from us within two weeks, please call the office and request your results.
Prescriptions will be filled during business hours and may take up to 2 business days to process. Please leave a message for our medical assistants and they will return your call by the end of the following business day.
Our providers are only available during normal business hours. If you have a medical emergency, please go to your local emergency center. If you have an emergency that has to be addressed after hour, call 615.395.6918. Listen to the message. When prompted, choose the selection:

- “To speak to Dr. Price urgently”
- “To speak to Dr. Valet urgently”
- “To speak to Dr. Roffwarg urgently”
